Lost paper from optician? Watch

I went to get a checkup at the optician but I lost the paper he gave me that was a description of my sight etc.

Where can I get access to the diagnosis of my sight? Is it stored on the NHS system so do I just talk to my GP? Or should I contact the optician and ask to give it again if they have a copy of it stored?

Optician. They cna probably read it out iver the phone if you are competent at taking notes or e-mail you a new copy.

They'll still have it on their system. Just give them a ring. They may not be able to give you any information over the phone due to confidentiality, but they may. You could probably ask for a copy of it to be sent to you
