If I need to send a separate personal statement for the chemistry course, can I use parts from my medicine one i.e extracurricular activities, or do they both have to be completely unique?

Your med extra curriculars aren't likely to be very relevant to chemistry. The people reading it will have access to both statements so you would usually be better off sending a shorter statement that is 100% chemistry focused.

I think that'd be little like sending a generic cover letter for a job application. You'd be much better off tailoring your PS for chemistry, rather than do a cut-and-paste job.
