Just whenever you move in. If you already live there just write today's date, I guess.

If (for example) you were moving to Southampton to stay with friends/not in halls, and already knew your address, you might want to put it on there with a 'valid from' date.

Yes your smart id and student id cards are the same thing. it is one card which will hold your free first year uni link bus pass, also your sport rec card if you apply for one and access to the bicycle sheds on campus if you use those too.

Valid from date is the date you moved to the address, it's not really critical it's more if you don't live there yet but it will become your permanent address (say you're in the middle of moving house) so like I might put 1st October 2007 as the valid from date.

I've heard some accommodation areas have their own gyms, are we able to use them with our student cards if we pay the £87 for the main one?

I believe you'll need SportRec for both.

You do need Sportrec even if you only want to use your Hall's gym (or at least you did last year, as I checked).

Small Fitness Suites have been set up in two Halls of Residence for use by residents only.

These facilities are managed locally but users must be members of the SportRec membership scheme and have following the Fitness Induction procedures.
