What degrees can I do with maths, biology, chemistry and English literature? I want to do medicine but worried it's too competitive, are there any other alternatives considering my choices?

You could go for Biomed undergrad then GEM, though that is quite a difficult route to go down from what I know. Could also look at Pharmacy/Pharmacology...

But your A-levels don't limit you from some degrees, you could go on to do Accounting & Finance... (though I guess this is more if you don't apply to medicine, rather than looking for a 5th uni course)

Good luck with your med application though and do all you can now to make it as competitive as possible.

Biomedicine (like me!), medical sciences or if you want to work in the NHS do Healthcare Sciences (where you specialise)

You could even try your luck at 2-3 med schools and apply for 2-3 of the courses above, so even if you get rejected from the med schools you'd still have a choice about where to go

Any biological or medical science degrees keep your options open career wise if you're not sure about medicine.
