Got an interview for a healthcare position and have no idea what to wear, they said dress smartly but I'm not sure, do you think this is appropriate?

The skirt is certainly fine and the shirt is a 'maybe'. If they have told you to dress smartly then I would say that you also need a jacket.

    Best of luck!

I think that's fine as long as you tuck the shirt in. Since it's winter it'd be good to have a blazer or something but you don't want to spend a fortune so if you don't have anything don't worry too much.
