Iv cannula issues

what are the complications someone used iv cannula in improper way ?
Failure, brief pain, subcutaneous insertion and subsequent submit administration of drugs or fluids, infection which could lead to cellulitis or abscess formation, thrombophlebitis, bruising, arterial insertion if you go digging.

It depends how they used it, what did they do? Did they try to take blood from it or put a very heavy medication through it like TPN, which should only be administered through a PICC line?
