maybe they drunk too much fluid's before coming to school or filled up too much at the water fountain or something.

Even as an adult there has been times on the rare occasion where i've been to the toilet and left the house ...and then suddenly need to go within about half an hour again. I'm sure it happens to everyone once in a blue moon, but as most people arn't banned from using the toilet we don't wet ourself.

As for your breaktime keep the kids in for half of breaktime or until the last 5 minutes, and then go.

If you carry on with your policy eventually a kid is going to wet themselves, it's only a matter of time..........
Well I can count on one hand the amount of children I have permitted to go to the toilet, never had a parent complain or an accident. The longest a child would have to wait in my class is 2 hours not unreasonable in my view
How are you feeling about your exams?
