Is there any point putting maths 5th choice with AAAA at alevels considering its competitive so it would look bad as 5th? While loads of people will have had A*s

As a 5th choice for medicine? Your question isn't really clear. And what subjects have you got?

Although I have not seen this before and can't give medicine-specific advice, I think all you would have to do is research and find a uni that is happy with an A in maths, and also a uni that you would be happy at instead of a random one.
