I am currently in Year 12 and have completed the D of E Silver Award. I am thinking of starting the Gold Award but I am thinking that it will take up too much time from my A-level preparation. I will be volunteering at a care home and be doing some medical shadowing anyways (to gain work experience) but if I start the award I'll need to also do 1 year of the other activities required. Will having the award make a significant difference in my medicine application, especially if I have sufficient work experienced and have volunteered extensively?

I don't think it will, no, especially if you have sufficient work experience and volunteer work. It's more what you gain out of these things rather than having to extensively list things on your personal statement. I feel like you could gain a better insight into things through your voluntary work/work experience and besides, DofE Silver will have given you plenty that you can reflect on as it is, I'm sure.
