Particularly for "higher" roles, can my doctor act as my personal and/or professional reference?

Additionally, can they also be a reference for post-grad applications?

How well do you know your doctor?!

Normally for applications, it is better if you have professional or academic references. You can ask your personal tutor for a reference, and a reference from work experience/employment/volunteering. Try not to use your family doctor for a reference.
