How realistic are target grades and predicted grades. Are they generally pessimistic/optimistic or accurate?

It depends. How accurate do you want them to be? Predicted grades are what you make them, so start working hard and you'll see them improve, honestly. Don't be down about your predicted grades as they are extremely easily improved.

Well, my predicted were 8A* and 2A. Got 7A* 1A 2B in the end. But many of my friends improved from their predicted so I'm the odd one out :P
My "target" grades (or what I hoped to get once I left my last exam) was 3A* 4A 2B 1C....

My target grades were all B's
I was predicted mostly A* and a few A's and a B
I achieved 2A*, 8A, 2B and a D
Combination of exam stress and over predictions tbh. My target grades came from all sorts, like my year 6 attainment, post code, income- nothing actually related to efforts or academia!!
