    i see a therapist regularly, who told me i show a lot of signs of ADD/ADHD, and i too have been feeling like I have it for a long time. I got a letter from the hospital about a month ago with a questionnare for my parents to fill out. the letter also said that they'd sent a questionnaire to my school as well.

    anyway, my parents filled out theirs (they are fairly certain i show a lot of signs for ADHD/ADD as well) and sent it back. i recieved a letter today from the hospital saying they had not heard back from my school with the questionnaire they were supposed to fill out. in the letter it said
    "we have received the completed questionnaire from parents, but not from the school. we are assuming the school do not feel that an ADHD assessment is required for ******, and the referral has therefore been declined"

    i feel that it's kinda unfair that the hospital is making the decision just on the school's view - why didn't they take into account my parent's questionnaire and the information my therapist (a trained mental health nurse) provided. surely they should take it all into account, and not just the school?

    what am i supposed to do now?

    It doesn't sound like the school completed the questionnaire at all. I would contact the medical office and ask about it.
