pharmacy exam question; emergency supply and digoxin

What would you do in this scenario?
a patient came out of hospital last month with a letter asking her to double her dose of digoxin by taking 2x tablets. She tried to get an additional supply from the surgery but they would not supply it as they had not received this letter from the consultant yet and she was not due another prescription. She comes in and asks for an emergency supply.

(Not a pharmacist myself so i could be wrong )
Call the surgery yourself and get the story straight and verify why they haven't issued a prescription themselves (the patient might not always give you the full story) and ask the patient if they are able to take a copy of that letter in themselves to get the prescription issued more quickly then go from there.

Think about the legal requirements for emergency supply (see relevant chapter in MEP) as well as the clinical reasoning - is it at the request of the patient or the prescriber, have you interviewed the patient, what is the legal classification of digoxin, can the consultant supply a prescription, are you satisfied that the dose is safe / intended, is it a narrow therapeutic window drug, are you happy to double the dose without knowing the most recent digoxin plasma level, is there a reason why she cannot go to an OOH GP?
