I told my PhD supervisor from the start (had to, to explain my poor track record thus far!) and she's been hugely supportive. I'm very lucky though - I know other lecturers in the department wouldn't have been as patient.
Generally though I think if you feel able to tell someone this, you should! You're protected by the Equality Act 2010 and it can be good for uni tutors to know

I wouldn't personally, but it probably does help.

My uni disability officer created a support plan that he emailed to all of my professors explaining I may need to leave lectures/seminars early without notice due to mental health

I did and I would again, I'd hate the idea of them thinking that I wasn't meeting deadlines or not working to my full potential just because I was lazy or anything
