    Hello,

    I have received the EU17B form to fill in but I have a couple questions regarding the evidence I need to provide.

    1.My mum was changing jobs in 2015 (the tax year that I need to provide evidence for) so up to 2015 June/July she was in one workplace then unemployed and started a new job in October. Should I send the payslips from both workplaces separately?

    2. For the unemployment period- she didn't receive any support so do I need to provide evidence for this period (separately state that she received no money) or just leave it out?

    3.Should the evidence from the workplaces be translated and signed by a person of good standing? Or just the photocopy is enough? (My parents work in Lithuania)

    Hi Monika,

    In answer to your questions on the EU17B , If your mother had 2 employers we would require evidence of all her income for 2015.

    2. I would suggest that you send letter confirming that your Mother was unemployed for a period and received no support .

    3. The evidence should be in the form of an employers letter on company headed paper stating their yearly earnings from 1 January to 31 December. This does not need to be translated.
