There are lots of studies on this. This link would give you an idea of how to answer the first part. For the last question, you should focus on how doctors can abuse their authority.

So you have to define what social change involves. Then try and link that to how the practice of medicine should be carried out. Bring this together with the analogy of a tool and how that might not fit with the most ethically correct use of medicine.

It's a pretty wishy-washy question, so in the context of an exam with relatively high stakes and time pressure I would choose a different question.
