DSA evidence for mental health issues

Hi, im applying for DSA for next year and I have a few questions.

1) I'm going into my final year, am i still eligible?

2) What kind of evidence is needed? I have a letter from when I saw a psychiatrist for an assessment who diagnosed me with anxiety, depression and BPD under the acute mental health team but it doesn't mention how this affects my studies - does it need to? If so would I need to ask my GP for a letter of this? I haven't seen my GP in months since my mental health has not been under their care.

Yes, you are. I had DSA from the start of my degree for physical health issues, but towards the end of my second year I had a "top up" and got MH additions (specifically a specialist MH mentor).

It does need to explain how it affects you (although I don't remember if mine did tbh, this was back in 2015 for me). It might be worth emailing your university to see what they say?

Yes you can.
A note from your GP giving a basic explanation of what's going on. It doesn't have to be super specific cos you'll be having a needs assessment. Your GP should be fully capable of doing that as long as they are aware of your condition (psych should be updating them so they will be). The letter from your psych would probably be fine too as long as it's recent.

You can always ask your uni support team for help with this. They will be pros at it by now.
