Should be extremely simple if you know that you can rewrite this equation in vector form as

rn=d \mathbf{r} \cdot \mathbf{n} = d

where n\mathbf{n} is your perpendicular vector to the plane, and r=(x,y,z)\mathbf{r} = (x,y,z) vector.
