From what I've gathered a 'U' isn't a grade, and therefore doesn't need declaring on UCAS.

But I'm not sure if it has to be mentioned on the Cambridge form.

How are they gonna know...

I dunno, can they find out?

From what I remember, the Cambridge form asked for grades of all exams taken. If you've taken it then maybe you should put it on. That being said, I left off an AS Ethics and Philosophy module on the advice of my school because it wasn't going to lead to a qualification. I got an A in it, so it wouldn't have been detrimental. Not sure why they told me to leave it off to be honest.

"UCAS say: 'You must enter all your qualifications where you have accepted certification – even if you failed or got a low mark'"

No it does not need to be mentioned on UCAS, I'm 100% sure of that. A 'U' is not certificated.

But whether it needs to be put on the Cambridge form, I'm not so sure.

Yes you do need to put it on. If you are made an offer and they find out you haven't disclosed it, they have the right to revoke your offer.

Just put it on the form.
