What does it mean exactly by the phrase ‘I think therefore I am’ and why does Rene Descartes take on this view?

Even when applying Descartes' method of radical scepticism, we always know that we are thinking. Thus we cannot doubt our own existence, because even the most radical doubt requires thought. As such, 'the proposition, I am, I exist, is necessarily true whenever it is put forward by me or conceived in my mind' (Med. II)

Materialism is a form of monism, yes. Descartes is, of course, famously a dualist.

I saw an example question that was along the lines of evaluate body and soul to the ‘Four Causes’, but how do they relate?

I'm not the greatest Aristotle scholar so I'm not really sure, but I guess Aristotle would think of it like this:

First cause (material): our bodies

Second cause (formal): soul

Third cause (efficient): God, but also your parents

Fourth cause (final): contemplation of god

But as I say, Aristotle is not a major area of interest for me, so you might want to check (or perhaps somebody better versed than I on Aristotelian philosophy can chip in here).
