What does 'I don't know what I would do' mean here?
'I care so much about you. If anything happened to you I don't know what I would do.'
They wouldn’t know how to cope without you ie they care a lot about you

It means they would not know what to do (How to act, what to think) if anything happened to you, by anything they are probably referring to injury, serious injury or the worst, death. If someone says this to you it means they really care about you.
Simply means they care alot about you.
