I’m currently working at grades 5/6 in my subjects will grade 7s be achievable if I revise hardcore from now ???

Yes anything is possible, if you put the effort. Just do lots of past papers/questions for all subjects, but you should've started revising long time ago & hardcore? what happened?

They could absolutely be achievable! Try your best, and set up a revision timetable to cover all the things you want to cover to get there.
But also giving yourself time for breaks and social things is important and can help just as much. There'll be times when your brain just can't take any more in at that moment, and that's okay! Take a break, and come back to it later. Sometimes you need to step back and come back to it with a refreshed mind. Ask others for help too, they might have a cool way of remembering something that you haven't thought of which can absolutely push that grade the extra step.

I did revise a lot for sciences but I guess just generally demotivated and worried
