Wherever you go, the qualification is the same so it's down to your personal preference on things like places to live and teaching/learning styles, as well as considering which university would be the best chance of interview for your academic and personal profile.
You don’t choose your specialty until s looong way down the line, you do a medicine programme first and specialise later, although you could choose your intercalation option to be most applicable to neurosurgery to start down that road
