    Can anyone explain the attached reaction and give a mechanism for it?

    It's a conjugate addition, I think - so you'd have to do it at around 80 degrees to avoid getting the direct addition product (attack at the carbonyl).
    CN- attacks the alpha-beta unsaturated part, pushing electrons up onto the oxygen. The carbonyl then reforms and the electrons go towards pinching a proton from the alcohol.
