I have completed my bronze duke of edinburgh, but do not know where my certificate is, would ucas ask for it if I write about it in my personal statement?

Universities aren’t really interested in DofE awards. Unless you did something specifically related to your subject then it’s not a good use of limited space in your PS.

DofE isn’t a qualification.
