Grade-wise you sound realistic, but nobody can tell you your chances of getting in because grades are only one part.

A couple of bad modules doesn't make your application a write-off. Do you know whether you came in the top 10% or 5% of your year?

I'd say every stage of the application is important including the CV. It's your chance to demonstrate relevant skills you've developed/relevant positions you've had. Although I doubt they'll expect too much from it since you're going into third year rather than someone with multiple years of professional experience.
