What should a social smoker say on their medical form

So for uni I need to register with a GP in that area and one of the questions on the medical form regards smoking status. I'm not one of those 'social smokers' who are low key addicted; I probably have a couple of cigarettes a month if the people i'm with at parties are smoking.

The 3 options are 'current smoker' 'ex-smoker' and 'never smoked'

Which option should I choose? I was also thinking about creating a fourth box to check instead explaining my situation?

I’d probably put never smoked. They’re really on about people who smoke as a habit or have smoked a lot

'Current smoker' is the most accurate.

Yes but since it asks for no further details that makes it appear as if I smoke daily. I meant what I said about how little I smoke; for reference, I think the last time I smoked was about a month ago.

You are still a smoker and it is still bad for your health.

I'd say "never smoker" is the closest option.

Current smoker. But put a little asterisk by the box to explain maybe.

Just put current smoker, as it's definitely "not smoked" and you're likely going to continue smoking like you are now,

I'm a medical doctor. It doesn't really matter what you write, to be honest. It's only really relevant to us if you have any type of breathing problem to better understand the cause.
