How likely am I to get into Medicine?

I got predictions: A* in physics A* in maths A in chem A in bio and achieved an A in Arabic. I also achieved a good ucat score: 730 in DM, 710 in QR, 680 in AR and a 600 in VR because I only started english in 2016 so I didn't know how to read quickly- an average of 680 (Top 17%).I applied Kings, UCL, Cambridge
Should I take a gap year or apply a 5th choice?
Are they rational choices?
Do I have a bad chance of getting in?
