question about DSA

Hi,

So I decided to apply for DSA and I got the forms through this morning. I've read the notes that came with it but I'm still stuck on filling it out. When in section 6 it says
please give full details of the nature of your disability, mental health condition or specific learning difficulty
, what exactly does that mean?

Also, for the evidence you have to provide, would that be just a letter confirming your diagnosis or how it affects you as well?

It's been a while since I last filled it in. I think you just need to give the exact diagnosis, that's all. So for a learning disability then dylsexia, for a mental health condition bipolar disorder etc.

As for the letter then it just needs to confirm the diagnosis. It doesn't need to go into any detail about how it affects you (as this will be dealt with during your needs assesment). Just a simple confirmation of diagnosis is fine. For a learning disability this would be a psychologsts report, for anything else a doctor's letter.

Ok, thanks. I wasn't sure whether I'd have to write down its effects on my studying but yeah that makes sense if they do it during an assessment - otherwise there might not be enough room on the form! lol

I just sent in a letter from my specialist - then i got asked for more information - i dont know if everyone does - but then i got a letter from my specialist explaining things in a bit more detail.
rather than start a new thread i thought I'd just ask here.

In section 5 of the DSA form it asks for university verification of the data you've submitted. I don't start uni until september, how do I go about getting this?

I don't think you fill that section in, it's only for distance learners. I think you skip to section 6.

Section four ends with, "if you are a full time undergraduate who does not study by distance learning go to section 6.

I've done it before, it took me about five minutes of reading the form before realising myself! I hate forms.
