On my 6th form application it asks if i have any mental health conditions, I have anxiety however would that mean that if I say that I do have a mental health issue, then they wouldn't consider my application?

Are you diagnosed? And even if you do put it down it's nothing to be ashamed about. They won't not consider you're application they're probably just asking so they can arrange something that will help you

Yes, I was diagnosed with social anxiety disorder and generalised anxiety disorder

The best thing to do will be to put it on the form. They just want to help you

Just be honest and put it down on your form.
You will still be considered , it's just something they have to do.
