Discuss means you can talk about anything within that section so it does not just have to be about the topic in the question ie. Poverty is the main cause of poor health, this question would mean i could talk about who poverty is the main cause in one paragraph and also talk about 3 other factors that influence poor health and state why although poverty is a more prevalent factorTo what extent would mean how important is poverty in causing poor healthIronically they mean the same thing almost

Ok that makes sense so basically just do the same for both but word it slightly differently.
