In my AS I achieved:

B - Psychology
D - Biology
U - Chemistry

But my teachers have predicted me:

A - Psychology
B - Biology
C - Chemistry

This is because I did well in my mock exams before AS, but struggled during the AS exams due to personal reasons. My teacher has mentioned these personal reasons on my reference, but is it still compulsory for me to put my AS grades onto the UCAS application?

Yes, as is stated on the UCAS webpage, all grades have to be declared:

“You must enter all your qualifications from secondary education onwards – whether you have the result (even any that were ungraded) or you’re still awaiting exams and results.”

It depends whether they were actual external AS exams. If they were external exams they have to be shown. If they were internal school exams they dont need to be shown.
