I am on my last year of a Level 3 BTEC Extended Diploma aged 17, I will be 18 by September and want to start another BTEC Level 3 in health and social care, will it be free or would I have to pay?

Yes it's free, as long as you start it before you turn 19 the course will be free

Although courses are free for those under 19 on enrolment, if you already have a full Level 3 qualification, then you are not entitled to funding for another course. Some colleges MAY let you enrol, but they don't have to, so you will need to check with your tutor.
