HELP with Student Finance form

Hi,
I'm struggling to finish my student finance form and I am wondering if anyone can help me. Basically, I'm stuck on the "previous addresses" section where it wants me to list all my previous addresses between 1 September 2015 to 1 September 2020. I put two addresses on there. So the first one from 2015 - 2018 and the second one from 2018 - now but I changed it to 2020 for the "to date" since that was what the question asked me. However, it still kept telling me that what I entered was wrong or there were overlaps or it told me to another address but I've only lived in two addresses from 2015 - now.

can someone help me please to make it more clearer for me.
