GOS1 form

Do I have to fill in this form every time I go to specsavers? Do I have to bring proof that I'm entitled to a free eye test even though I've been there before?

Yeah, I have to fill it out every time I go, even when I went to Moorfields Eye Hospital. I do take evidence with me as well.

My ESA Entitlement letter because it shows them I am on Income Related ESA, which entitles me to free eye tests.
