Hello!

I have a problem with the Certifier Checklist. I know many EU students have the same problem from what I have seen but I still have not found out how to sort it out. The notary who certified the copy of my ID and passport said that she could not give an official stamp on the Certifier Checklist. I have also asked other places and they said the same. Is there any other option as I do not want to send my original ID because I will be travelling to the UK soon so I need to have these with me.

Thank you!

Hi Katarina,

A certified copy is a clear photocopy of an original document which has been stamped, signed and dated as being a true copy of the original by an official.

You can download the certifier checklist here http://ow.ly/d/8nhW

To be valid the following details must be provided either on the certifier checklist or on the same page as the Identity evidence;
The certifiers full name (Must not be a relative)
The certifiers signature (Must be in ink)
The date of the certification (Must be dated within the last 90 days and prior to form being received)
The certifiers job title (Must be an acceptable occupation)
The certifiers work address
An official stamp
All pages of the Identity evidence, including the certifier checklist if provided must show the same official stamp
Both sides of the National ID card must be visible
The Passport number must not be redacted or obscured

Thanks, Isaac

so if the person does not want to complete the certifier checklist, can he put the information behind the photocopy of the certified identity card?

No, it must be on the same page, not behind.
