What are these forms / what do I do with them?? (For the NHS g's)

So this is a question for the NHS dons, because I'm guessing they're the only ones who know about this stuff. So I got some mail today, seemed a bit dodgy from the get go because it was opened and then sellotaped back shut, but anyway.. It's an NHS "questionnaire" (as the title says) with no introduction or anything, but it's split into different questions. There's:

PHQ-9, GAD-7, IAPT Phobia Scales, Work and Social Adjustment Scale, IAPT Employment Status and Risk Assessment.

Bunch of rando questions with no envelope to send it anywhere or phone numbers... what's it about (like, why am I being given one, I've had no contact with a GP or anyone) and once I've filled it out what on earth do I do with it?
(Only asked on TSR because I couldn't find anything on reddit or quora lol, they're like my life gurus) Thanks all.
(P.S. I don't know how to put this in the right place so... I'm sure healthcare uni students are relevant here anyway)
