Do you get a hard shoulder, lay by and refuge area on a carriage way, dual carriage way and motorway? where do those 3 things live?

Hard shoulder is on the motorway, the dead lane to the left used for stopping in an emergency and for BMW/AUDI drivers to undertake poor people in heavy traffic.

Refuge area on a dual carriageway (and smart motorway), basically a large lay-by normally filled with truckers and swingers, and most of the time has a barrier to protect it from passing traffic.

Lay by on a single carriageway, a place to park and have picnics, sell cars or discard smutty magazines.
