It enters the blood. Lymph is just interstitial fluid, protein and fat.

Does it go to the heart via the vena cava?

Lymph enters the venous system via the subclavian veins. Once in the blood, it mixes with the blood. From the subclavian veins, blood travels into the brachiocephalic veins, which unite to form the superior vena cava which enters the heart.
