For a UCAS application you should definitely have an academic reference, usually it would be the responsibility of your form tutor (under whatever title they have), as they have an objective overview of your progress across various subjects. Remember they aren't allowed to say anything bad (I've written about 300 of the things), but negative aspects can be highlighted by exclusion - in other words, a lack of mention of attendance and punctuality skills
