Vaccine Attitudes and Knowledge Questionnaire!

Hi everyone,

I am investigating knowledge and perceptions towards a paediatric vaccine (the 6 in-1 vaccine). For this, I am distributing a questionnaire which shouldn't take longer than 10-15 minutes.

This is for my final-year project so I would really appreciate it if you could take part!
