Migrant Worker maintenance loan application form

So I found out that I’m eligible to apply for a maintenance loan as an eu migrant worker, however I’m in the middle of the academic year and am not sure which form I should be using? Can anyone please tell me which form I need to fill in? And when and how please help ;(

Hi, please can you first advise us of when your course started and the exact date you entered the UK?

My course started on the 1st of October in 2019 and I entered the UK on the 1st of August 2018. I’ve been working for 4 months.
