How to get means tested

Hi, I haven't applied for a maintenance loan at all this year so I haven't been means tested by student finance. However I do qualify for a bursary from my university, but in order to get that I've been told I need to be means tested by student finance. Apparently to do so, I need to fill out two forms, one to let student finance know I want to be means tested and the other to provide them with my financial information. I think the second form is a PFF2 form which I've found online, however I can't remember what the other form is or where to find it.

Sorry if this is a vague question, any help is greatly appreciated. Thank you!

Not sure if I'm meant to mail it in or somehow submit it online, but I'll try filling it out first. As for the "other form" that I vaguely remember, I think I'm just gonna have to ring them up again and ask, yippee.

Thank you, I'm sure that's the correct form so I'll fill these in asap.
I think you would put zero there and then say unemployed in the next question, and if you get employment later on you might have to contact them again and change it. I don't think it would affect your bursary though, if you received it and then became employed (but you might want to check this with someone else lol)
