I got an interview coming up and I have reschedule it once due to family circumstances and now I have an important appointment which clashes my interview and I don't want to look bad to the university, so what shall i do?

Could you have the appointment moved instead?
Otherwise, explain your situation to the university. It's out of your control, so they shouldn't be bothered.
