Anyone has any idea on whether transfers happen the other way round where people go from London to Oxbridge? Or whether it is possible to transfer within the London schools?

Not possible in either case as far as I'm aware.

It used to be technically possible to transfer into Cambridge for the clinical years (from any medical school, not specifically London). I'm not sure if it still happens. It was not commonly done.

You can still transfer from Oxford to London atm, but I think that opportunity will end for any future incoming students.
