Do you want to become a doctor, or not? That's the key question. If you want to become a doctor at any point then turning down a place on a medicine course is a bit pointless. If you don't want to become a doctor then you should turn down the medicine place and do whatever it is you would rather do instead (which may or may not involve applying to Oxbridge).
