Trying (and failing) to study over summer. Help?

I'm going into year 11 in September and I planned to revise everyday and work on some subjects where I got lower than I wanted on my mocks, but it's been a week and a half now since I finished school and I've done nothing. I have no motivation, which isn't helped by my not so great mental state, and I just don't know where to start or how to stick to a plan because let me tell you I am horrendous at sticking to routine and healthy habits etc and I could just do with some help so if you can I'd appreciate it. Ty

I am also very bad at sticking to a routine. I manage to overcome it by making a checklist of all the things I want to get done as opposed to creating a revision timetable. Then I just do the tasks whenever I have time until I'm finished

Thanks, I might do this instead as timetables just stress me out like I like to plan in advance but I can't cope with planning every thing to the second yk what mean and I feel like I'm almost restricted with a timetable like if something comes up I'm completely thrown off
