Attempt past paper questions with notes. Eventually, you'll notice yourself not needing to go back to your notes. Also, high chance your notes aren't well? Maybe try understanding your notes but most of the time doing questions will make you understand your notes in more depth.

Good news is that you have set an achieveble goal
Why do you think your performance is low? Is it due to gaps in your knowledge? Do you think you've poor reading comprehension when it comes to understanding what the question is asking of you? Etc.

Write flash cards from the course spec
Then practice past papers bc the wording is very specific it’s bad a memorisation game
Jabchem has lots of past papers and miss adams chemistry on yt shows simple calculations
Doing past papers with out the required in isn’t very useful so learn some course from flash cards first and make sure you can do the problem solving ones
Also to boost your grade make sure your project is decent
