This feels like an unlikely increase in grade. I don’t say that to be mean but I’d really encourage you to have a plan for if you don’t get a grade 6. Are you being entered for higher tier?

You probably want to do a mix of past papers and topic based questions. I’d start by doing the first half of a foundation past paper and then mark it. Any questions you get wrong, go away and practise that topic. Then repeat the process. When you’re getting most stuff write do a full foundation paper, then move on to the first half of a higher paper.

Good luck!

It sounds like if you're struggling that much you might need a tutor.
With the time you have it might be difficult to jump that high, that being said the best chances to improve in my mind would be to get all the past papers you can, even if some of them are for a different exam board. Work through them one by one in exam style conditions. Then go through them and mark them with someone who knows their stuff and get them to show you how they would do the questions you got wrong. Then repeat with the next papers. As you're correcting your mistakes you'll learn how you should do it and should find your marks slightly improving with each paper. Don't be afraid to ask lots of questions if you don't understand what's being explained.

Alot of the techniques you use will be similar paper to paper. Good luck.
