You can use a planner, calendar, blank piece of paper, etc… to plan out your revision timetable.
There are many styles to plan with - this depends on what you feel is easy to read and understand.

Example:
Monday April 29
9:00 - 10:00 Biology mitosis & meiosis revision
11:30 - 12:00 English Lit practice paper Q2/3
…

Tuesday April 30
7:30 - 9:00 Nuclear Physics
2:00 - 2:30 DT revision on thermosetting and thermosoftening
…
And so forth…
